Easy Use Guide

Finding and downloading legal forms is very easy.

First, locate the form you need by using any of the links on the home page or by using the search engine.

Second, click on the ORDER icon on the line with the form control number.  This will add the form to your shopping cart.

Third, continue shopping to add more forms by using your back button or the return links, or click on the "Submit this Order on the Secure Server" button at the bottom of the shopping cart.

Fourth, complete the secure order form and click "Process my Order - Secure". This will take you to the download page where the available formats of the form will be listed.

Fifth, right click on the desired format of the form and save to your hard drive.

Sixth, open the form, edit, complete and print.

Seventh, don't worry if you have problems. Simply E-Mail us and we will assist. Send E-Mail about orders to

    Order/Download Procedures:  Users may generally order/download using one of the   following procedures:

    1)    Regular Users - As a regular user you may locate the form you desire, click on the order icon, complete the secure order form and obtain the form.

    2)    Registered Users - As a Registered User you may order and download forms using a username and password. To join as a registered user you place your contact information and credit card information in the USLF Registered User system.

    3)    Fax Orders - You may order forms by fax.  To use the fax procedure use the Regular User procedure and simply print and fax rather than submitting the secure order form or use the fax order form. The fax number is 1-(601)-825-2102.

    4)    Phone Orders - You may also order by phone toll free 1- (877)  389-0141. Phone orders are taken Monday-Friday, 8:30 a.m. - 5:00 p.m., Central Standard Time.

    5)    Mail Orders -  You may also order by mail with payment by check,money order or credit card. 

    Payment Methods:  The following methods of payment are accepted: Visa, Mastercard, Discover, American Express, E-Check by phone and Regular Check by mail. To use the E-Check method please follow this procedure:
    1)    Locate the form you desire.

    2)    Call toll free 1- (877) 389-0141, Monday-Friday, 8:30 a.m. - 5:00 p.m., Central Standard Time.

    3)    The form will be provided to you by E-Mail.
